The Wellness Coaching Questionnaire is designed to accurately evaluate the holistic function of the body and to assist one in beginning their Wellness Coaching treatments. Proper treatment comes from a proper history.

Upon completing this questionnaire, Centra Chiropractic can evaluate your health care needs and provide a Diagnosis and Report of Findings. The Diagnosis and Report of Findings is a charged service.
